The link below will take you to the NGJA/NCAA Judges Evaluation Form. The purpose of the form is to provide coaches feedback on judge's performance during an NCAA competition.
Directions: Use the link below to bring up the form. Fill in the general meet information, the names of the judges, and check off the boxes as necessary. If you need to make comments, use the comment section at the bottom of the form. Please remember to include the name of the judge to whom the specific comments refer. If you would like to comment on more than 6 officials, please use a second form.
